Information & Communication Policies
Practice Communication Policy
We offer telephone consultations in line with current legislation, meaning patients must have been seen in-person at the clinic within the previous 12 months to be able to receive a Medicare rebate. Otherwise, the telephone consultation is a completely private fee.
If you need to communicate with your Doctor, please email the reception staff on email@example.com and they will manage communication with your Doctor. Doctors do not normally take phone calls while consulting.
Management of Patient Health Information Policy
All patient health information is considered private and confidential and therefore not disclosed to any third party without the patient's written consent.
Individuals can request access to their health information verbally or in writing and consent is given by their treating GP for this access.
For any complaints, please call 3211 3611 or email the practice manager directly at firstname.lastname@example.org
If you are not satisfied with the response, please contact the Office of The Health Ombudsman on 133 646 or www.oho.qld.gov.au
